Richard M. Cadle, PharmD, BCPS, FASHP  

Richard M. Cadle, PharmD, BCPS, FASHP is the Pharmacy Practice Residency Director and the Clinical Pharmacy Manager. He received his PharmD from the University of the Pacific College of Pharmacy in Stockton, California and then went on to complete a pharmacy practice residency at the VA Medical Center in Phoenix, Arizona. He serves on the HIV team and the Infection Control Committee. Dr. Cadle is also involved in several investigator initiated research protocols, and he serves on the manuscript review board for the Annals of Pharmacotherapy and the American Journal of Health-System Pharmacy. He is an Assistant Professor of Medicine at Baylor College of Medicine. He is also a clinical assistant/associate professor at University of Texas, University of Houston, and Texas A&M Colleges of Pharmacy. He is an active member of Gulf Coast Society of Hospital Pharmacists and past president.

Sima Desai, PharmD   Sima Desai, PharmD is the IV room supervisor. She received her Doctor of Pharmacy from St. Louis College of Pharmacy. She has worked at two other VA facilities before coming on board at the MEDVAMC in 2009. She previously served as the evening shift supervisor before obtaining her current position.

Harold Doucet, PharmD   Harold Doucet, PharmD, originally from Opelousas, Louisiana, received his Doctor of Pharmacy from Texas Southern University in 2003. He currently serves as the Outpatient Pharmacy Supervisor as well as the Methadone Pharmacist for the Substance Dependence Treatment Program. His background includes 23 years of Federal Service in the Military on active Duty as a 91Q20Y7 (Pharmacy Specialist,SGT,IV trained). After leaving active duty, he started his 20 year service in the pharmacy department of the Michael E DeBakey VA Medical Center beginning as a technician, pharmacist, and recently joining the pharmacy administration team.

Brian R. Fase, PharmD   Brian R. Fase, PharmD, MS, originally from Grand Rapids, Michigan, received his degree from Ferris State University in Big Rapids, Michigan. He recently completed the MS / PGY-1 / PGY-2 pharmacy practice management program at the MEDVAMC and is now the MS / PGY-1 / PGY-2 residency director and the pharmacoenomist.

Veronica Franklin, BS   Veronica Franklin, RPh is the Chief of Pharmacy Services. She received her BS in Pharmacy from the University of Louisiana at Monroe College of Pharmacy. She has held several positions throughout the VA system, including inpatient clinical specialist at the Sonny Montgomery VA Medical Center in Jackson, Mississippi and staff development and quality management specialist at the MEDVAMC. She currently precepts students and residents during their practice management experience of the program.

Thomas Huston, BS, MS, MHSA   Thomas Huston, BS, MS, MHSA is the inpatient pharmacy supervisor. He received his BS and MS from Oregon State College of Pharmacy and his MHSA from Arizona State University. He then completed a pharmacy residency at Harris Methodist Hospital. He has previously served as Assistant Chief at the West Los Angeles Veterans Affairs Medical Center.

Cheryl Mitchell, PharmD   Cheryl Mitchell, PharmD graduated from Texas Southern University College of Pharmacy. She has served as a staff pharmacist, assistant outpatient pharmacy supervisor and now serves as the outpatient pharmacy supervisor. Her areas of interest include outpatient pharmacy performance measures and quality improvement.

Vy Nguyen, BS   Vy Nguyen, RPh is the staff development and quality management pharmacist. She received her BS from the University of Houston College of Pharmacy. Following graduation, she worked at Wal-Mart Pharmacy for three years where she was the pharmacy manager as well before joining the MEDVAMC in 2001. Her experience at the MEDVAMC includes inpatient, research, and oncology. In 2010, she successfully completed the Veterans Affairs Leadership Development Program.

Tai Vu, BS   Tai Vu, RPh is a clinical pharmacy specialist in drug procurement and informatics. He received his degree from the University of Houston College of Pharmacy. At the MEDVAMC, he is actively involved in all human subject protocols that require medications. He also serves as a non-voting advisory member for the Research and Development Committee at MEDVAMC. He is trained in inpatient pharmacy practice and procedures, which include intravenous admixture, bar code medication administration, and the Pyxis system.
